Parent Contribution
Dear Parent(s)/Carer(s)
By completing and returning this form, it is understood that as a parent(s)/carer(s) you have read and understood the following terms and conditions. A copy of my GDPR privacy policy can be found under policy.
THE ASSESSMENT PROCESS:
That Emily Vincent will undertake an educational psychology assessment with your child with the aim of providing an independent evaluation of your child’s strengths, difficulties and recommended strategies. Assessments will include one-to-one work with your child and may include an observation in class (if Emily thinks it is appropriate) and discussion with school staff and other professionals involved in your child’s care. The structure, pace and manner of each assessment varies depending on the needs of the child /young person.
Factors that impact on your child’s education will be considered and commented on if deemed to be relevant, including developmental history, medical issues, home life, social & emotional skills and behavioural observations. Your child will also be asked their views.
In order for me to undertake the assessment, parental consent must be provided by one parent. Please ensure that other individuals with parental responsibility are made aware that the assessment is taking place, so that they may contribute to the process if they wish to do so. In addition, the completed report should be shared with them.
When completing the parent form you will be required to provide the name and contact details of other professionals that have been involved with your child, and by submitting the form you give permission that Emily can contact these professionals regarding their involvement if further information is required (unless you state otherwise).
That Emily will contact your child’s school (unless specifically requested not to in the space provided) and request that the relevant staff/teacher completes a School Information Form. This will ask teachers’ views of the child’s current situation, educational attainments and details of what support has been provided to date etc.
An hour feedback meeting will be offered usually on the day of assessment, or telephone feedback, to discuss the outcome of the assessment.
With agreement to these terms and conditions by submitting this booking form, there is a deposit to be paid of £400. This is deducted from the final assessment fee (please see further information under Fees/cancellations).
Please note that I can carry out assessments that contribute to the EHCP process, however I do not attend tribunals.
FEES:
Please refer to my list of assessments and related fees sent at the time of enquiry/booking.
The balance is due before or on the day of assessment. The fee and payment details will be provided in the confirmation email sent to you on receipt of this form. The report will not be released until payment has been received.
The fee covers: the time it takes to consider the background information; prepare the assessment; assess the child; provide feedback and write a report including a summary of the findings and recommendations; at least eight hours of time. If a case is more complex than first communicated, further hourly charges (over the eight hours) may apply.
The fee does not cover telephone conversations (other than for feedback post assessment) international telephone calls or mileage costs/travel time (please refer to my assessment/fee list). I reserve the right to charge for additional phone calls.
If additional assessments/scales are required, further fees apply (please refer to my assessment/fee list).
Should an assessment be cancelled with less than two weeks’ notice, or if the child fails to attend their appointment*, the deposit will be non-refundable. For assessments scheduled during the Easter or Summer school holidays, a minimum of four weeks’ notice is required for cancellations. Without this notice, the deposit will also be retained.
*Should your child feel unwell for their appointment, please inform me by email as soon as possible. If we are able to reschedule the appointment within a few weeks of the original date, your deposit will remain unaffected.
THE REPORT:
- Details of the assessment will be provided in a written report, which is usually sent at least 21 working days after the assessment day. At busy times this may be longer; it is not always possible to complete reports before this time frame, especially when assessments take place close to school holidays.
- Please be aware I work the equivalent of four days a week and do not work weekends or during state school holidays.
- I would encourage you to take notes during the feedback session if you need to know strategies/details before that time frame.
- Please allow 21 working days before following up on the progress of your child’s report and before booking any follow up meetings/appointments.
- To avoid the report landing in your junk folder, please ensure you have saved my email and added it to your safe contacts: Emily.Vincent@edpsych-glos.co.uk
